Believing that children are a wonderful gift from God, St. Paul’s Church is committed to being the best possible stewards of this gift.   St. Paul’s Safe Church Policy is a key tool in fulfilling this important stewardship responsibility.  The primary goal of the Safe Church Policy is to (1) ensure the safety of all within St. Paul’s U.C.C. and (2) be in compliance with the PA State laws regarding volunteers and employees having direct contact with children. This includes children and adults participating in (1) activities within St. Paul’s facilities and (2) activities sponsored by St. Paul’s U.C.C. at off-site locations.

Forms that volunteers having contact with children are required to have on file at St. Paul’s include:

For Adult Volunteers (18 years old and older):

(1)  V1 - PA State Police Request for Criminal Record Check (SP4-164A)

(2)  V2 - Child Abuse History Certification from Dept of Human Services (CY113)

(3) V3 - Adult Emergency Contact Form

Additionally, a fingerprint based Federal (FBI) Criminal History Report is required if the volunteer has lived outside the Commonwealth of Pennsylvania in the last 10 years.  This process must begin on-line at

For Youth (Under 18 years old):

V4 – Volunteer Application and Disclosure Form for Minors (VADFM)

            SP1 – Confidential Report of Suspected Incident of Child Abuse
